- The hourly fee is $100.00. This is payable at
the time of service. Checks, cash, or money orders are acceptable
payment in the office. If you pay in advance, you may use a
credit card or debit card via PayPal on the internet. Talk with
Vivian if you have need of other arrangements.
- If your health insurance covers outpatient mental health, Vivian will accept the co-pay + co-insurance fees from you, and file a claim for the balance on your behalf to your insurance company.
- If you have not covered your annual health insurance deductible for the current year, you will have to pay out of pocket, and Vivian will file insurance so that your payment will be applied to the required deductible.
- Vivian Carol is a BCBS and BCBS PPO provider. If your policy covers outpatient mental health, you will pay only your co-pay and co-insurance fees, assuming you have covered your annual deductible.
- For insurance carriers other than BCBS, check your insurance policy to determine if they cover "out of network providers". If so, they will usually pay a percentage of the fee and you are liable for the balance.
